The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring, direct care, and collaboration with the care team. This role assists community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ideal candidate demonstrates flexibility, teamwork, and a positive, professional approach while working in a fast-paced environment.

Key Responsibilities:
-Administer insulin and other medications that QMAPs are not permitted to administer.
-Assist with follow-up documentation for incidents, changes of condition, and other progress notes.
-Follow up with primary care providers regarding medication status, order clarification, and updates.
-Work under the direction of the DON and ADON to support the Wellness Department.
-Occasionally work as a QMAP as needed to ensure medication administration coverage.
-Assist in coordination of the resident admission process, including screening, assessment, and identification of care needs.
-Ensure care is delivered as indicated in each resident’s care plan.
-Administer medications and treatments per physicians’ orders.
-Coordinate care and services with outside providers, including resident physicians and therapy partners.
-Communicate proactively with residents, families, and POAs regarding care updates, changes in condition, and follow-up needs.
-Document and report care provided, resident observations, and any unusual or significant changes.
-Maintain a thorough working knowledge of state regulations, policies, and procedures.

The Maintenance Technician assists the Maintenance Manager with executing the general operating maintenance and repair of the community and physical plant. They work to maintain proper operations of the community in a safe, clean, and comfortable manner.

In this position, your main responsibilities will include:

• Respond to resident and family requests for maintenance assistance.
• Must be capable of following direction to make electrical, plumbing, HVAC, painting, and structural repairs.
• Working outside on exterior tasks including irrigation, tree and vegetation requirements, pool maintenance (if applicable), hardscape surfaces, and roofing.

Salary: $65,000 - $73,775 annually

In this position, your main responsibilities will include:

• Recruitment, selection, training, supervision, and staffing of team members.
• Implementing resident-centered care.
• Provide nursing care to residents, including but not limited to: Completion of health assessments and resident care plans, discussing changes in resident condition with physicians, introduce interventions to reduce falls based on individual resident needs, etc. Also provide medication administration and help with resident care, as needed
• In conjunction with the community’s multidisciplinary team, coordinate the resident admission process including screening, assessment and care planning.

This position may occasionally fill in on the weekend as a Manager-On-Duty.

ESSENTIAL JOB FUNCTIONS:

• Provides strategic direction for the community to provide adult-centric, relevant, elevated and purposeful programming that addresses the Physical, Social, Spiritual, Emotional, Cognitive and Leisure needs of our residents.

The responsibilities for this role include:
