In this position, your main responsibilities will include:
- Providing residents with assistance in the activities of daily living such as grooming, bathing and maintaining their personal environment.
- Transporting residents within the community to activities, other departments, or dining.
- Responding to emergency calls, resident and/or visitor requests, and environmental hazards in a timely, appropriate, and safe manner.

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.
- Assist in coordination of the resident admission process including screening, assessment, and care needs
- Ensure care is delivered as indicated in resident care plans
- Medication and treatment administration and ensure medications are administered per physicians’ orders
- Assistance with coordinating care and services with other outside providers including resident physicians
- Document and report care provided and resident observations including unusual or significant changes
- Maintain a working knowledge of state regulations, policies, and procedures
